Our expert Team of Occupational Therapist’s understand complex care within continuing healthcare (CHC). When someone becomes CHC eligible they will inevitably have needs of complexity. This will be in relation to their combined health and social care needs which require an accurate follow up assessment by an OT who can offer recommendations in relation to equipment needs, manual handling and care and support needs. When equipment is assessed and prescribed by an OT this can mean an improved quality of life for the person and maintain an individual’s independence. This could include provision of bespoke equipment, assistive technology or simply altering technique.
Accurate assessment, positive risk taking and provision of appropriate equipment, assistive technology and/or manual handling techniques ensures a package of care is optimised for the person while giving mutual consideration to funding, facilitating the best outcomes for all concerned.
Regular reviews of equipment are required for best practice to ensure all provided equipment continues to meet a person’s needs and should be clearly outlined to care givers and providers that any changes in a person’s support needs, need to be reported to the funding body so that all equipment can be reviewed. Typical changes include weight gain or weight loss as this may affect the type and size of sling that is being used or pain and discomfort.
Complex needs are about Individuals who require or are in receipt of complex care (also known as long-term care or continuing care). People receiving complex care have substantial and ongoing healthcare needs so changes can occur as needs change. These can be the result of chronic illness, disabilities or following hospital treatment.
