• Formal written assessment’s.
• CHC Checklist completion and submission to CCG/CSU.
• Decision Support Tool completion (DST)
• MDT attendance to represent client needs for DST
• Evidence of need provided for DST to include obtaining and collating all other professional assessments reports
• DST roof reading
• CHC eligibility Review’s (3 month and 12 month)
• CHC retrospective assessment of need
